Because I am focusing on young people under the age of 16, I need a parental consent for your views to be collected online. The parental consent can be obtained directly from me via my official university email address e.keryova@sussex.ac.uk or verbal consent can be done via telephone 07876701703. I will allocate to you a unique identifying number which you'll need at the beginning of the survey.

Please note, this is just an example of the parental consent form. Your version will be in Word/PDF format and it will contain the unique identifying number.
